Financial Policy

We require payment in full on the initial visit regardless of insurance coverage. We are glad to file your claim for reimbursement as a courtesy to you. We are unable to accept temporary checks. We will address payment methods for your surgery during your consultation appointment.

PAYMENT OPTIONS:

  1. Cash payments: We offer a 5% discount for surgery charges over $400 if you pay the balance in full with cash or check on day of surgery.

  2. Insurance: With insurance approval, pay 1/3 of total balance (deductible and non-covered procedures not included in 1/3 of balance) on day of surgery. Insurance is a contract between you and your insurance company, and it will be up to you to know what your insurance covers. We will do our best to estimate your out of pocket expense.

    We are glad to file your insurance claim as a courtesy to you, or will provide you with the information needed for you to file. We participate with the following companies: Highmark Blue Shield PPO.

  3. Highmark Blue Shield: Requires that we collect your deductible plus your co-insurance percentage when surgical charges are incurred.

  4. Delta Dental Insurance: Requires that we collect, depending on your coverage, either your balance in full or, if Delta will reimburse our office directly, your deductible, co-insurance, and amount of the charges over Delta’s fee schedule when surgical charges are incurred.

  5. United Concordia Dental Insurance: Requires that we collect, depending on your coverage, either your balance in full or, if UCCI will reimburse our office directly, your deductible, co-insurance, and amount of the charges over UCCI’s fee schedule when surgical charges are incurred.

  6. Extended Financing: We offer extended financing through CareCredit. Please feel free to contact our office for more information. Applications must be completed prior to use in our office.

  7. Service Charge: Accounts are subject to monthly service charge after 60 days, regardless of pending insurance (Does not apply to Blue Shield and Delta Dental claims).

Third Party Litigation: Litigation (lawsuits) can sometimes take years to settle. We do not bill attorneys for charges incurred for motor vehicle accident related injuries or injuries incurred in an accident where a third party is involved. The patient or parent/guardian is the party responsible for payment in full. If there is prior approval from auto insurance or medical insurance, then our office will bill to the appropriate insurance. We will provide your attorney with the necessary medical records to settle your claim.
